What a skills assessment template is for
A skills assessment template is a structured sheet for scoring the capabilities a role needs against the capabilities the people in that role currently demonstrate. The goal is not a number for every person; the goal is a short, ranked list of gaps worth closing.
Most templates fail because they mix too many ideas in one row, use a 1-5 scale with no behavioural descriptors, or leave the evidence column blank. The template below fixes all three: one competency per row, a 0-4 scale with plain-language levels, and a required evidence method.
The 0-4 competency scale
Five levels, 0 to 4, with behavioural descriptors. Resist half-points; the precision is imaginary and it slows scoring down.

The template columns
One row per skill. The weighted-gap column does the prioritising for you: gap × criticality. Keep the skill list between six and twelve per role.

How to keep the assessment honest
- Score one skill at a time across all people, not one person at a time across all skills. The comparison set stays fixed and the scores become more consistent.
- Back critical skills with evidence, not manager impression. A short assessment, work sample, or observation produces a number someone else could reproduce.
- Record the evidence source in the sheet. A current level without a source becomes a guess within one quarter.
- Re-assess the same skill with the same instrument. A different question or different rater makes improvement impossible to prove.
From the template to continuous assessment

What scale should a skills assessment use?
A 0-4 scale with behavioural descriptors. It has a real midpoint, avoids false precision, and makes scoring almost mechanical once the descriptors are agreed.
How many skills should be assessed per role?
Six to twelve. Beyond twelve, managers cannot hold the model in their heads and scoring quality drops.
Who should score the current level?
For low-criticality skills, the line manager. For critical skills, use an objective instrument such as a short assessment, work sample or structured observation.
Can I use this template in Excel or Google Sheets?
Yes. The CSV opens in any spreadsheet. The columns are plain text so you can sort, filter and share without reformatting.
