Skills Assessment Template: A Ready-to-Use CSV and a Scale That Actually Works
A skills assessment template turns a vague list of competencies into a scored grid you can act on. Here is a free CSV with the columns that matter, a 0-4 scale with behavioural descriptors, and a worked example row.
What a skills assessment template is for
A skills assessment template is a structured sheet for scoring the capabilities a role needs against the capabilities the people in that role currently demonstrate. The goal is not a number for every person; the goal is a short, ranked list of gaps worth closing.
Most templates fail because they mix too many ideas in one row, use a 1-5 scale with no behavioural descriptors, or leave the evidence column blank. The template below fixes all three: one competency per row, a 0-4 scale with plain-language levels, and a required evidence method.
The 0-4 competency scale
Five levels, 0 to 4, with behavioural descriptors. Resist half-points; the precision is imaginary and it slows scoring down.
| Level | Name | What it means in practice |
|---|---|---|
| 0 | None | No exposure. Would not know where to start. |
| 1 | Aware | Knows the concept exists and where to look it up. Cannot do it unaided. |
| 2 | Assisted | Can do it with a checklist, a template, or someone to check the work. |
| 3 | Independent | Does it unaided at the expected quality and speed. The target for most roles. |
| 4 | Teaches | Handles the edge cases, improves the process, and trains others. |
The template columns
One row per skill. The weighted-gap column does the prioritising for you: gap × criticality. Keep the skill list between six and twelve per role.
| Column | What goes in it |
|---|---|
| Skill or competency | One observable capability, phrased as something a person does |
| Behavioural definition | The specific behaviour that proves the skill |
| Criticality (1-3) | How much the business hurts when this skill is weak |
| Required level (0-4) | Target for this role |
| Current level (0-4) | Evidenced where possible |
| Gap | Required minus current, never below zero |
| Weighted gap | Gap × criticality — your sort column |
| Evidence method | Assessment, call review, work sample, observation |
| Evidence source | Where the current level came from |
| Action | Train, coach, document, hire or accept |
| Owner | The person accountable for the action |
| Re-assess date | When you will measure the same skill again |

The row below shows how the template works for a support team. The weighted gap of 6 tells you this skill outranks others even when the raw gap is the same size.
| Skill | Definition | Criticality | Required | Current | Gap | Weighted | Evidence | Action |
|---|---|---|---|---|---|---|---|---|
| Handles a refund dispute without escalation | Resolves the dispute within policy, keeps the customer, and documents the outcome | 3 | 3 | 1 | 2 | 6 | Q3 assessment — 41% correct | Train with spaced repetition |
How to keep the assessment honest
- Score one skill at a time across all people, not one person at a time across all skills. The comparison set stays fixed and the scores become more consistent.
- Back critical skills with evidence, not manager impression. A short assessment, work sample, or observation produces a number someone else could reproduce.
- Record the evidence source in the sheet. A current level without a source becomes a guess within one quarter.
- Re-assess the same skill with the same instrument. A different question or different rater makes improvement impossible to prove.
From the template to continuous assessment
A spreadsheet is the right place to start, but the current-level column ages quickly. Metronic turns the template into a living score: upload the material a role depends on, generate questions from it, and the platform keeps asking the questions that matter until people reach mastery.
The result is the same grid, except the current level refreshes itself instead of waiting for the next survey round.

Frequently asked questions
What should a skills assessment template include?
At minimum: skill, behavioural definition, criticality, required level, current level, gap, weighted gap, evidence method, evidence source, action, owner and re-assess date. The downloadable template on this page includes all of these plus the 0-4 scale.
What scale should a skills assessment use?
A 0-4 scale with behavioural descriptors. It has a real midpoint, avoids false precision, and makes scoring almost mechanical once the descriptors are agreed.
How many skills should be assessed per role?
Six to twelve. Beyond twelve, managers cannot hold the model in their heads and scoring quality drops.
Who should score the current level?
For low-criticality skills, the line manager. For critical skills, use an objective instrument such as a short assessment, work sample or structured observation.
Can I use this template in Excel or Google Sheets?
Yes. The CSV opens in any spreadsheet. The columns are plain text so you can sort, filter and share without reformatting.
Try it with your own material
